Advance Directive Day
National Healthcare Decisions Day is April 16th! Have you completed your Advance Directive, Living Will, or Power of Attorney for health decisions?
We are providing these services to our community for FREE from 10:00 a.m. - 6:00 p.m. on April 16th!
We will provide you with all the documents needed. Just come by to meet with one of our notaries or social workers to help you with these decisions.
We will have representatives from the following agencies to assist you:
- Amheart Hospice
- BJC Home Care/Hospice
- Parkland Health Center
- Preferred Hospice
- Safe Harbor Hospice
